To process an entry visa to the Ukraine, please submit the following:
- US Citizens: Original passport (signed)
- Non-US Citizens: Original passport (signed)
- Copy of both sides of green card
- 1 Passport size picture
- A completed visa application (one per person), signed by the Applicant
